چکیده

L atent autoimmune diabetes in adults (LADA) is a term used to describe a form of autoimmune diabetes that resembles type 1 diabetes, but has a later onset and slower progression toward an absolute insulin requirement. Controversies have been surrounding this concept and several attempts have been made to better characterize and classify it. But LADA still remains poorly understood and defined (1). It was even debated whether LADA exists as a distinct disease entity or it just represents the end of a wide spectrum of heterogeneous immune-mediated diabetes (2,3). Uncertainties concern almost all aspects of this disease, including the nomenclature, diagnostic criteria, epidemiology, natural history, and pathogenesis with genetic, metabolical, and immunological aspects. As a consequence, there is no clear management strategy for it, in terms of therapy and prevention. An ideal therapeutic approach would aim not only at obtaining a good metabolic control, but also at protecting residual -cell mass and function. Even though 10% of adults with presumed type 2 diabetes at diagnosis in fact have LADA, only a few studies so far have evaluated therapeutic interventions for LADA, using a hypoglycemic or an immunomodulatory agent.

Characteristics and Prevalence of Latent Autoimmune Diabetes in Adults (LADA) in Torbat-e Heydarieh, Iran

Background and Aims: The prevalence of latent autoimmune diabetes in adults (LADA) among diabetic patients is less recognized and underdiagnosed. The aim of this study was to determine the prevalence of LADA in type 2 diabetic patients and to compare the characteristics of these two groups in Torbat-e Heydarieh, Iran. Materials and Methods: Totally, 198 male and 277 female patients diagnosed ...

The Time Is Right for a New Classification System for Diabetes: Rationale and Implications of the β-Cell–Centric Classification Schema

The current classification system presents challenges to the diagnosis and treatment of patients with diabetes mellitus (DM), in part due to its conflicting and confounding definitions of type 1 DM, type 2 DM, and latent autoimmune diabetes of adults (LADA). The current schema also lacks a foundation that readily incorporates advances in our understanding of the disease and its treatment. For a...

Pancreas-enriched miRNAs are altered in the circulation of subjects with diabetes: a pilot cross-sectional study

The clinical presentation of diabetes sometimes overlaps, contributing to ambiguity in the diagnosis. Thus, circulating pancreatic islet-enriched microRNAs (miRNAs) might be useful biomarkers of β-cell injury/dysfunction that would allow more accurate subtyping of diabetes. We measured plasma levels of selected miRNAs in subjects with prediabetes (n = 12), type 2 diabetes (T2D, n = 31), latent ...

Peripheral blood mononuclear cells of patients with latent autoimmune diabetes secrete higher levels of pro- & anti-inflammatory cytokines compared to those with type-1 diabetes mellitus following in vitro stimulation with β-cell autoantigens

BACKGROUND & OBJECTIVES Type-1 diabetes mellitus (T1DM) and latent autoimmune diabetes in adults (LADA) share similar pathological features but differ in age of onset and progression. There is a scarcity of information on differences in CD4+ T-cell responses, particularly, cytokine secretion, between the two forms of autoimmune diabetes. Here proliferative potential and concentration of pro- an...
